Patient authorizations are required for Hometown Specialty Services through CMS guidelines and the URAC and ACHC specialty pharmacy standards of care for every individual patient to be provided appropriate onboarding paperwork and consent.

REMs Medication Forms
REMS, or Risk Evaluation and Mitigation Strategy, is a program implemented by the U.S. Food and Drug Administration (FDA) for certain medications to ensure that their benefits outweigh their risks. These programs are designed to manage known or potential serious risks associated with specific drugs and are often required by the FDA to reinforce safe medication use practices.
For Brixadi & Sublocade Prescribers do not need to certify in the REMS we have supplied these onboarding forms to help comply for new providers. In advance of the patient’s appointment, send a prescription for the named patient to our certified REMS pharmacy. Our pharmacy will coordinate delivery with the patient’s scheduled appointment and deliver to the prescriber or the administering practitioner designated by the prescriber. BRIXADI & SUBLOCADE is never dispensed directly to the patient and must only be administered by a healthcare professional in a healthcare setting.
